Planning Board Application Information

Below you will find the Instructions, Application Form, and other papers required to be completed if you are requesting a hearing before the Avon Planning Board.

They include the following:

  1. Instructions
  2. Application Form to apply for a hearing
  3. A W-9 Form
  4. Notice of Hearing Form that can be used when notifying Property Owners
  5. Notice of Hearing Form that can be used when advertising in the Newspapers
  6. Affidavit of Proof of Service
  7. Applicants Check List ( this form is provided for your use and does not have to be returned)

Please read the instructions carefully as you must comply with certain time limits and guidelines. If these time limits and guidelines are not followed your application may be delayed and hearing date affected.

All hearings are to be done through Zoom so all documents submitted to the Planning Board must also be submitted in PDF form
